Physician disability insurance quotes
One request returns a side-by-side comparison from Guardian, Principal, MassMutual, Ameritas, and The Standard, quoted for your specialty and your state. No fee and no obligation.
A physician disability insurance quote should be a comparison, because the five major carriers class the same specialty differently and recognize specialties through different own-occupation mechanisms. Seaworthy runs Guardian, Principal, MassMutual, Ameritas, and The Standard simultaneously and reads the contract language beside the premium, at no cost and no markup.
What a physician quote comparison includes
Every request is priced by all five carriers at once, and the comparison lines the contracts up on the provisions that decide a physician's claim. Price is one column of the six. The rest of the table is contract language.
| What we compare | Across the five carriers | Why it matters for a physician |
|---|---|---|
| Own-occupation definition and specialty recognition | Whether the contract treats your medical specialty as your occupation, and the mechanism each carrier uses to recognize it. | Specialty recognition decides whether a claim is measured against your specialty or against medicine in general. |
| Occupation class for your specialty | The class each carrier assigns your specialty, which is a primary driver of the premium. | The same specialty can sit roughly two class tiers apart across carriers, so the cheapest carrier for one physician is often the wrong one for another. |
| Monthly benefit and issue limits | The largest benefit each carrier will issue at your income, and how employer group LTD or a GSI plan offsets it. | Determines how much of your income you can actually protect on top of what you already have. |
| Mental and nervous benefit structure | Whether full-benefit-period mental health coverage is available to your specialty, or a 24-month cap applies. | A capped policy stops paying a mental health claim after two years even when the disability continues, so the structure is worth knowing before you buy. |
| Residual and partial benefits | The trigger for a partial-disability claim and whether benefits pay on income loss alone. | A physician's claim often begins with reduced hours or a dropped procedure line, not a full stop. |
| Premium and premium structure | The quoted cost, whether the contract is noncancelable or guaranteed renewable, and which physician discounts apply. | Two policies at the same price can carry very different guarantees. |
The carrier-by-carrier detail behind that table, including how each of the five delivers true own-occupation and recognizes a specialty, is on the physician carrier comparison page.
Why physician quotes have to be run by specialty
Carriers price physicians by occupation class, and the class map is anything but consistent from one carrier to the next. Per the mid-2026 producer guides, an orthopedic surgeon is 5M at Ameritas but 3M at Guardian and Principal. A pediatrician is Principal's only 6M physician specialty. An ophthalmologist sits in a surgical class at MassMutual and a top medical class at Principal and Ameritas. None of that is visible on a single carrier's quote, and in our placement experience, class moves the premium more than most rider decisions do.
Specialty also shapes the contract itself. Most physician specialties can elect full-benefit-period mental health coverage, while anesthesiology, emergency medicine, and pain management generally face a 24-month cap under the same mid-2026 producer guides. The specialty-by-specialty class grid across all five carriers is on the occupation classes page, and our house ranking of the five carriers for physicians is in the best disability insurance for physicians guide.
What our placed book shows
In Seaworthy's 2026 audit of placed policies, about 26% of physician policies carry an exclusion or a rating, and mental and nervous conditions account for roughly 40% of those physician exclusions. The median age at issue for physicians in the book is 36. In our placement experience, applying while the health record is still clean is the most reliable way to avoid an exclusion, and a future increase option then grows the benefit with income without new medical underwriting. More on how physician coverage fits together is on the physician disability insurance hub.
How the comparison works
Tell us five things
Specialty, income, state, gender, and date of birth. Existing group or individual coverage details help but are not required.
We run all five carriers
Quotes are run simultaneously against your specialty's class at each carrier, with the contract provisions read beside the price.
You decide with the full picture
You receive the side-by-side comparison, usually within one business day. Nothing goes to a carrier until you choose to apply.
No fee, and no change to your price
Seaworthy is an independent brokerage. The agency is compensated by the carrier only when you decide to place coverage, so the comparison itself costs nothing, and the premium is identical whether you apply through Seaworthy or directly with the carrier.

Physician quote questions
Is there a fee for a physician disability insurance quote comparison?
No. Seaworthy is compensated by the carrier when you choose to place coverage, so the comparison costs you nothing. The premium is the same whether you apply through Seaworthy or directly with the carrier, because commissions are built into the rates state insurance regulators approve.
Which carriers are included in a physician quote comparison?
Guardian, Principal, MassMutual, Ameritas, and The Standard, the five carriers that, in the agency's experience, write among the strongest individual own-occupation coverage for physicians. When none of the five can cover a situation, the agency also has access to the Lloyd's of London surplus-lines market.
Do the quotes account for my medical specialty?
Yes. Every quote is run against your specific specialty, because carriers assign the same specialty to different occupation classes and recognize specialties through different contract mechanisms. An orthopedic surgeon, for example, is classed 5M at Ameritas but 3M at Guardian and Principal per the mid-2026 producer guides, a spread that changes both the premium and the contract that fits.
What information do you need to run physician quotes?
Your specialty, annual income, state, gender, and date of birth. If you carry employer group LTD, association coverage, or an existing individual policy, those details sharpen the comparison, since issue limits are applied net of coverage you already have.
Can you compare a GSI offer or review a policy I already own?
Yes. A guaranteed standard issue offer from an employer or training program goes into the comparison beside fully underwritten quotes, so you can see what each covers and what each costs. The agency also reads policies clients already own against the current market and flags where the contract is strong and where it falls short.
